What Is a Myers Cocktail IV?

A Myers Cocktail is an IV drip containing magnesium, calcium, several B vitamins, and vitamin C, delivered slowly through a vein rather than swallowed.

It's one of the oldest and most widely offered formulas in IV vitamin therapy, and it turns up on the menu at nearly every wellness clinic, med spa, and mobile IV service in the country, usually sold as an energy or immunity drip.

What almost nobody tells you is where the formula came from, or how thin the research underneath it is. So here's the version with the receipts attached: what's in it, what it may support, what the published evidence does and doesn't show, and the questions worth asking before you let anyone put a needle in your arm.

The Quick Answer

Where the Myers Cocktail Came From

The formula is named after John Myers, MD, a physician in Baltimore who spent much of his career giving patients IV vitamins and minerals as part of treating a range of medical problems, and who never published a formula or a case series describing what he was doing.1 When he died in 1984, his patients went looking for someone who would keep giving them the infusions they'd been receiving monthly or weekly, in a few cases for 25 years or more.1

Alan Gaby, MD, took many of them on, reconstructed a version of the mixture from what the patients described and what he could work out pharmacologically, and used it in his own practice for the next decade and change. In 2002 he published a review in Alternative Medicine Review laying out the modified formula, the rationale behind it, and what he'd seen clinically across approximately 15,000 treatments over an 11-year period.1

That paper is the reason the Myers Cocktail exists as a named product on thousands of clinic menus today, so it's worth being clear about what kind of paper it is. Gaby wrote it as a review of his own clinical experience alongside the published literature on the individual ingredients, and he was direct about the limitation: despite the anecdotal reports, only a small amount of published research supports the treatment.1 The man who wrote the founding document said so inside the founding document, which is more candor than most of the marketing built on top of it has managed since.

What's in the Bag

Myers' original mixture, as Gaby reconstructed it, contained magnesium chloride, calcium gluconate, thiamine, vitamin B6, vitamin B12, calcium pantothenate, B complex, vitamin C, and dilute hydrochloric acid.1 The modern version drops the hydrochloric acid and typically looks like this:

There is no single standardized recipe. Doses vary quite a bit between clinics, and so does whatever gets added on top. If a clinic sells you a Myers Cocktail, ask for the specific formulation, because the name covers a family of similar mixtures and not one fixed product.

Why IV Instead of a Pill

The pharmacology argument for IV nutrients is the strongest thing going for them, and it holds up on its own terms.

Your gut regulates how much of a water-soluble vitamin you can absorb. Push the oral dose higher and absorption efficiency drops off, so plasma concentrations flatten out no matter how many capsules you swallow. An IV skips that ceiling entirely.

Vitamin C is the cleanest illustration of this. In a 2004 study at the NIH, 17 volunteers received vitamin C by mouth and by vein at matched doses. A 1.25 gram oral dose produced mean peak plasma concentrations of about 135 µmol/L, while the same dose given intravenously produced about 885 µmol/L, roughly six and a half times higher.4 Scaling up from those measurements, the authors calculated that IV administration can reach plasma levels 30 to 70 times higher than the maximum tolerated oral dose.4

So the delivery claim is sound. IV really does produce blood levels that no supplement bottle can match.

What doesn't follow from that is the leap most IV marketing makes next. Having more of a nutrient circulating in your blood, when you weren't short of it to begin with, is a separate question from whether you feel or function any better, and it needs its own evidence.

What the Published Research Shows

This is where the story gets less flattering for the industry built around it.

The controlled trial. In 2009, researchers at Yale and Griffin Hospital ran what remains the only published randomized, double-blind, placebo-controlled trial of the Myers Cocktail. Thirty-four adults with fibromyalgia received either weekly Myers infusions or lactated Ringer's solution as a placebo over eight weeks, with the tender point index as the primary outcome.2 Most participants improved compared with their own baseline, and so did the placebo group, and the difference between the two was not statistically significant on any outcome measure.2 The authors concluded that the trial established safety and feasibility, while efficacy relative to placebo remains uncertain.2

The reference-text position. The Merck Manual's entry on IV vitamin therapy, fully reviewed in July 2025, lists the standard marketing claims (boosting immunity, raising energy levels, relieving stress, treating everything from migraines to fibromyalgia) and concludes there is insufficient scientific evidence that IV vitamin therapy produces any of them.3

The broader review. A 2023 review in Drug and Therapeutics Bulletin looked at IV vitamin injections in general and reported that the authors were not aware of any high-quality outcome evidence of health benefit for IV vitamins given in the absence of a deficiency or a specific medical indication.5

All three point the same direction. The formula is old and reasonably well tolerated, and after more than four decades of use nobody has run the trial that would show it does something specific for a healthy person.

So Why Do People Feel Better?

Plenty of people walk out of an IV appointment feeling noticeably better, and dismissing that would be its own kind of dishonesty. A few different things are probably contributing at once.

Fluid. Most clinics deliver a Myers in a bag of saline, and someone who showed up mildly dehydrated will often feel better within the hour of receiving it. That effect is real, and it's mostly the water doing the work.

Magnesium. IV magnesium has measurable physiologic effects, which is why hospitals use it for severe asthma exacerbations and for eclampsia. The warm flush people describe partway through a Myers infusion is the magnesium making itself known.1 Whether it does anything lasting for someone whose magnesium was already normal is a different matter.

Correcting a real deficiency. If you were low in B12, topping it up can change how you feel over the following weeks. The catch is that you only know you were low if somebody measured it.

Placebo, and everything around the placebo. The 2009 fibromyalgia trial makes this point better than any argument could, because the group getting saltwater improved too.2 Half an hour sitting still in a quiet room while a clinician pays attention to you does something measurable to how you feel, and that effect gets credited to the vitamins.

None of this is a reason to skip an IV if you want one. The point is just to be clear-eyed about why you're getting one, which is usually that you like how you feel afterward.

Where IV Nutrients Clearly Make Sense

Some situations call for IV nutrition without any hedging at all.

  • Documented deficiency, confirmed on bloodwork instead of inferred from how you feel
  • Malabsorption conditions, including Crohn's disease, celiac disease, and post-bariatric-surgery anatomy, where oral supplementation may not be reliably absorbed
  • Significant dehydration, where fluid replacement is the actual goal
  • Specific inpatient indications, such as IV magnesium during a severe asthma attack, which is ordinary hospital care1

Every one of those has something in common: somebody measured something before the needle went in. That's what separates IV nutrition as medicine from IV nutrition as an item on a menu.

Outside those situations, the framing that survives contact with the evidence is a modest one. An IV drip may support hydration and how you feel that afternoon, and a lot of people report exactly that. Research suggests it isn't a treatment for any illness, and it won't substitute for sleep or for finding out what's driving your fatigue in the first place.

Safety and Side Effects

Myers Cocktails are generally well tolerated, and the 2009 trial specifically established safety in its population.2 A few real cautions still apply.

During the infusion. Magnesium given too quickly can drop your blood pressure, which shows up as a wave of heat, lightheadedness, or occasionally fainting.13 That's why the drip runs slowly and why somebody should be keeping an eye on you. If a sudden warmth comes over you, say something to your nurse instead of quietly waiting it out.

If your electrolytes are off. In people with abnormal magnesium or potassium levels, infusions containing those minerals can cause arrhythmias or muscle weakness.3 Kidney disease matters a great deal here, since impaired kidneys clear magnesium poorly.

Drug interactions. Magnesium can compound the effect of blood pressure medication, IV calcium can interact dangerously with ceftriaxone, and high-dose IV B6 may blunt the response to erythropoiesis-stimulating agents such as epoetin alfa.3 Tell whoever is treating you everything you take.
