They work best if you're deficient, and that's where the evidence is strongest by a wide margin. If your level is already comfortable, the trials haven't shown much, though the trials also weren't designed around most of the reasons people actually walk in for one.
Which turns this into a question about you rather than a question about B12. And the answer runs backwards from what you'd expect. The people most likely to buy a B12 shot are, on average, the people least likely to need one.
The ones who actually need it are usually somewhere else entirely. They're 72 and their stomach stopped making enough acid a decade ago. They're on metformin. They've been taking omeprazole since 2019. They're six years out from gastric bypass. They are not browsing a wellness menu on a Saturday. They're tired, and they've decided that tired is just what their life is now.
The Quick Answer
Vitamin B12 injections correct deficiency, and correcting a documented deficiency may support a meaningful improvement in fatigue, since profound tiredness is one of the hallmark symptoms of being low. In people whose levels are already normal, a 2021 meta-analysis of 16 randomized trials and 6,276 participants found no effect of B12 supplementation on cognition or depression, and so little fatigue data that the authors couldn't run the analysis.1 Roughly 3.6% of U.S. adults are deficient at a serum B12 under 200 pg/mL, and about 12.5% fall into the insufficient range under 300 pg/mL.2 Knowing your number tells you which of those groups you're in, and B12 has no established upper intake limit, so the shot itself carries a favorable safety profile either way.
Your Liver Is a Doomsday Prepper
Start with how little B12 you need, because the number is genuinely funny.
An adult needs 2.4 micrograms a day.2 Not milligrams. Micrograms. That's less than you'd get from a single serving of salmon, and it's the entire daily requirement for a molecule your nervous system cannot function without. Only 5% of American men and 11% of women take in less than the estimated average requirement from food.2
Now here's what your liver does with that. It holds a standing reserve of 1 to 5 milligrams, roughly a thousand times your daily consumption.2 Your liver has been quietly stockpiling B12 for years against some catastrophe it has apparently been anticipating since you were a teenager. It has a three-year supply of a vitamin you can get from eggs.
The practical consequence is that you cannot become B12 deficient in a hurry. It takes years of something going wrong before symptoms surface. Which means when someone finally does turn up deficient, the interesting question isn't what they've been eating. It's what's been quietly failing at the absorption end the whole time.
The Three Jobs
B12 does three things worth knowing about: it helps build red blood cells, it maintains the myelin insulation around your nerves, and it participates in DNA synthesis.
Run short and those exact three systems fail, in that order of visibility. The red blood cell problem shows up as anemia, which is where the fatigue and the pale skin come from. The myelin problem shows up as numbness and pins and needles in the hands and feet. Somewhere in the mix people also report brain fog, palpitations, and a tongue that feels raw.
This is why B12 deficiency gets misdiagnosed as ordinary tiredness, ordinary aging, or ordinary stress. Every symptom on that list has forty other explanations.
Why the Energy Story Survives
The energy reputation is a true story told to the wrong audience.
Deficiency-level fatigue is heavy and real. Fix the deficiency and it may support a substantial improvement, sometimes a startling one.2 Someone who lives through that tells everybody they know, understandably, because from the inside it feels like discovering a cheat code.
Then the story escapes its context. It gets repeated to the roughly nine in ten adults whose B12 was never the bottleneck, and for that group the trial evidence is genuinely thin. A 2026 review in Nutrients went looking specifically at supplementation in people with no proven deficiency and found benefits concentrated in narrow slices, such as people with elevated homocysteine or levels parked at the low end of normal.3 Markun's 2021 meta-analysis arrived from a different direction and landed nearby. Across 16 trials and 6,276 participants: nothing on any subdomain of cognitive function, nothing overall on depression, and so few trials bothered to measure idiopathic fatigue that no pooled analysis was even possible.1
But "the trials didn't show it" and "it does nothing for you" are different statements. Those studies were mostly built around cognition and mood in older adults, not around how a 41-year-old with a level of 290 pg/mL feels on a Wednesday. Low-normal is a real category, the Nutrients authors flag it themselves, and nobody has run the trial that would settle it.
The one claim that does fall apart is weight loss. Mayo Clinic's position is that unless your levels are low, B12 injections aren't likely to give you more energy or improve your performance, and they are not a weight loss treatment.4
Who's Actually Low
These categories are large, and most people don't know they're in one.

Look at that list again and notice what almost none of it is about. Diet shows up once. Everything else is a plumbing problem, and plumbing problems are invisible until you measure them.
One group missing from the table: anyone eating dramatically less than they used to on a medical weight loss program. Total intake drops and B12 goes down with it, and a good share of those patients are on metformin too. That belongs on a lab order rather than in the guessing pile.
The Pill Math
The oral absorption numbers look catastrophic until you look at them for another ten seconds.
Through the intrinsic factor pathway you absorb about half of a dose under 1 to 2 mcg. Push the dose up and the percentage collapses: roughly 2% at 500 mcg, and 1.3% at 1,000 mcg.2 Which sounds like a disaster. 98.7% wasted.
Then you do the arithmetic. 1.3% of 1,000 mcg is 13 mcg, which is more than five times your daily requirement. And that fraction gets in by passive diffusion, which doesn't need intrinsic factor at all, meaning it still works in the exact people whose intrinsic factor has stopped working.
This is why the 2018 Cochrane review found high oral doses of 1,000 to 2,000 mcg normalized serum B12 about as well as intramuscular injections did.6 Worth holding that finding loosely: it rests on three small trials, 153 people total, three to four months of follow-up, and the authors graded the evidence low quality for exactly those reasons.
So the honest case for the shot is a practical one, not a pharmacological one. You show up once instead of remembering a tablet every morning for a year. The dose is certain. Absorption stops being a variable you have to think about. Those are real advantages and preferring them is completely rational. Injections also keep a genuine edge where absorption is the broken part: pernicious anemia, post-surgical malabsorption, severe deficiency.2
So Who Should Get One
Three groups, roughly.
If you're in the table above, this is the easiest call on the menu. You have a documented reason to run low and a cheap, safe way to address it, and your provider can help you decide whether oral or injectable fits your situation better.
If you've tested and you're sitting in that 200 to 300 pg/mL band, you're in the territory the research hasn't settled. The Nutrients authors name low-normal status as one of the few places benefits do show up.3 Trying a shot and paying attention to how you feel over the following weeks is a reasonable experiment, and your own results are better evidence for you than a meta-analysis built on somebody else's population.
If your level is comfortably normal and you want the shot anyway, that's a fine choice to make with clear eyes. B12 has no established upper intake limit and your body clears what it doesn't use, so the risk side is about as low as it gets. Just know what you're buying. The published evidence for a boost in that scenario is thin, and anyone promising you one is going past what the research supports.
Knowing Your Number
Get a serum B12. It's cheap and routine. If it lands in that gray zone between 200 and 300 pg/mL, a methylmalonic acid or homocysteine test can show whether the shortage has reached your cells or is just an unimpressive number on a page.
